ABOUT TFHIT..

The Twin Falls Health Initiatives Trust, Ltd was formed and organized to operate exclusively for charitable, scientific and educational purposes including, but not limited to identifying, selecting, and investing in, as appropriate, programs that will improve health and wellness of citizens of Twin Falls County, Idaho. It is also charged with identifying, selecting and providing grants to nonprofit charitable organizations that provide health and wellness related services in Twin Falls County, Idaho. The Twin Falls Health Initiatives Trust is committed to actively promoting and supporting educational programs that enable residents of Twin Falls County, Idaho to improve their health and wellness.

In the pursuit of this mission, the Twin Falls Health Initiatives Trust must prepare at least every three (3) years, a community plan to prioritize and address the health and wellness needs of the residents and community of Twin Falls County, Idaho. This document shall guide the Twin Falls Health Initiatives Trust Board of Directors in prioritizing and authorizing the expenditure of grant funds during the period covered by the current community plan. Each Community Plan shall be based on a community needs assessment prepared prior to developing the plan, and shall be approved by the Twin Falls Board of County Commissioners.

The Board of Directors of the Twin Falls Health Initiatives Trust distributed surveys to both residents and health providers within Twin Falls County, and then collected and analyzed the data. Based on this needs assessment and an extensive strategic planning session, the Board has drafted this community plan.
